Awakening from anesthesia is not just about drug dissipation. In this issue, Chemali et al.  show in animals that the monoamine transport inhibitor methylphenidate arouses animals anesthetized with propofol. This finding highlights the need for future studies designed to understand how propofol blocks sensory input (red arrow  ) and suppresses the widely distributed brain mechanisms (gears  ) regulating arousal.
